I have
an endowment LIC Policy No 222211113(for a sum assured of Rs 75,
000/-) with the LIC, Jeevan Nagar, Delhi

I had
contended my sister, Mrs Shriya Panday, in my policy, which I took
on 4 May 2010, as my sole nominee However, due to some family
issue, I have decided to change my nominee to Mrs Kamla Shastri,
resident of 34 K P Colony, Gwalior

I
would be highly obliged if you do the needful to change the nominee
as soon as possible My policy is due to mature on 5 May
2012
