@Bhawna, Namra has answered correctly. Answers for remaining query are as follows.

FAD: Flavin adenine dinucleotide

FADH:  When hydrogen gets attached with FAD. It is called FADH. 

H stands for hydrogen.  It is a semi reduced form of FAD. It is also called semiquinone.
