why did Albert felt nervous when he met the doctor?

Albert needed a false certification from  Dr Ernest, to enable him to quit school for six months. Einstein, however, when came round to meeting the doctor, suffered from an actual nervous breakdown--perhaps from an overbearing sense of guilt or falsity, as he was apparently lying to the doctor, about an ailment he did not have.
