You can take the format of a questionnaire as provided in your study material and you can use following questions while preparing your questionnaire.
* Do you check the MRP before buying any item?
* Do you check expiry date before making purchase of medicines, cosmetics or any other products?
* Do you check weight of the products mentioned on item?
* Do you compare price of a product at different markets?
* Have you come across adulteration in milk or any other consumable item?
* Do you check for ISI or Agmark while making purchase?
* Have you ever complained shopkeeper or supplier about some deficiency in goods or services?
* Was your complaint to the supplier/shopkeeper attended by him to your satisfaction?
* Are you aware of consumer courts, for redressal of grievances of consumers?
* Have you ever filed a case in the consumer court?
* If yes then was consumer court able to redress your grievance?
* Do you think media play an important role in creating consumer awareness?
