Its an idiom. Both neem fruit and bittergourd(karela) are bitter. Here Ranga is talking about the type of girl he wants to marry. He says that one can't marry a girl who has milk stains on one side of her face (very young that she drinks Milk) and wetness on other (who cries).And one who doesn't even know how to bite her fingers. ( too young👶 or an infant) Thus both these situations are worse for ranga to marry the girl. That is both these situations are bitter.
