telomeric DNA is not exactly 100% replicated during each replication cycle and this natural property of a cell. now the main function of telomeres is to protect the end of the chromosomes from action of exonucleases as with each replication cycle telomeric sequence gets shortened so after a point these won't be able to protect cell's own DNA from it's own enzymes and DNA gets damaged which beyond a specific point when DNA couldn't be repaired by DNA repair mechanisms it will elicit a apoptotic response which will lead to cell's own programmed death.
