During notemaking, is it compulsory that the subtitles (points) should be present in the paragraph?
for eg: 
""Deadlines are missed and the work flow is not only inefficient but of poor quality.""
Is it okay to write the point as "missed deadlines" ?

The sub-topics have to be taken from the given paragraph in that the point should have been mentioned. But, it is not necessary to use the exact same words. You may use your own words that convey the same meaning. In the given instance, 'missed deadlines' is correct as a sub-topic.
