Transgender is the term use to describe people whose gender identity or gender expression differs from biologically assign sex.
Normal male or female posses 46 chromosome. Transgender condition is formed due to sex chromosomal abnormality. The abnormality occurs due to non disjunction (is the failure of the chromosome pair to get separated during the meiosis type of cell division) of chromosome during cell division. Thus these people have increased chromosomal ratio than the normal people.

Syndrome Chromosome number  Ratio of chromosome Which Sex chromosome is abnormal
Triple X syndrome 47 XXX X chromosome

Most transgender people are men or women. But some people don't neatly fit into the categories of "man" or "woman" or “male” or “female.” For example, some people have a gender that blends elements of being a man or a woman, or a gender that is different than either male or female. Some people don't identify with any gender. Some people's gender fluctuates over time.

People whose gender is not male or female may use many different terms to describe themselves. One term that some people use is non-binary, which is used because the gender binary refers to the two categories of male and female. Another term that people use is genderqueer. If you're not sure what term someone uses to describe their gender, you should ask them politely.

It's important to remember that if someone is transgender, it does not necessarily mean that they have a "third gender." Most transgender people do have a gender identity that is either male or female, and they should be treated like any other man or woman. NO a transgender cannot reproduce.
